If you’re interested in becoming a provider or vendor to PAASP member/partner organizations, please complete the Provider Qualification (PQ) form.  Providers of training can include:  colleges/universities, technical schools, private companies, other employers, etc

Vendor/Provider Payment Process
 

1. In order to be qualified as a provider for PAASP, please complete the Provider Qualification (PQ) form
2. Immediately following completion of PAASP (approved by Deb Hines) training program, please mail, fax, or email Training Vendor/Provider Invoice to address below.
   Invoice to include:
   a.    Total Training Cost (as per agreed to amount with PAASP)
   b.    Dates of Training
   c.    Total Hours of Training
   d.    Contact Information
3. When required information on training participants is collected from employer/organization that sponsored the training, training vendor/provider invoice will be processed for payment.
